Liability/Media Release

Liability: I, being aware of my own health and physical condition, and having knowledge that my participation in any exercise program may be injurious to my health, am voluntarily participating in physical activity through Wheel Serve NC programming.

 

Having such knowledge, I hereby acknowledge this release, any representatives, agents, and successors from liability for accidental injury or illness which I may incur as a result of participating in the said physical activity. I hereby assume all risks connected therewith and consent to participating in said program.

 

I agree to disclose to Wheel Serve NC any physical limitations, disabilities, ailments or impairments which may affect my ability to participate in said physical fitness program.

 

Media Release:  I hereby grant and authorize Wheel Serve NC the right to take, edit, alter, copy, exhibit,  publish, distribute and make use of any and all pictures or video taken of me to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration.
